To participate in the Weis Feedback Survey, you will need:

- A computer, tablet, or smartphone with an internet connection.
Survey Weis Code
- A recent receipt from Weis Markets containing a 4-digit store number, 6-digit transaction number, and the date of your visit.
Knowledge Languages
- Basic knowledge of English or Spanish, as the survey is available in both languages.
WeisFeedback Survey Questions
- About 10-15 minutes to answer all the survey questions.
Valid Survey Details
- A valid email address to enter the sweepstakes and receive notifications about the survey results and future promotions from Weis Markets.
Weis Survey Restrictions
- Legal residents of the United States.
- Participants must be 18 years or older to take the survey.
- Only one survey entry is allowed per person per month.
- The survey must be completed within seven days of the date on the Weis Markets receipt.

- Choose your preferred language (English or Spanish).
- Enter the location Reward Card Number Date of Visit Time of Visit.
- Click on the “Start” button to begin the survey.
- Answer all the survey questions based on your recent shopping experience at Weis Markets. Be honest and provide as much detail as possible.
- After answering all the questions, provide your contact information, including your name and email address, to enter the sweepstakes.
- Click on the “Submit” button to complete the survey.
